2017/2018 Registration


School Registration Information

 The following documentation is required to register a student in a Gilbert Public Schools:

 Birth Certificate - This must be a state or county issued original or certified copy.

Proof of Residency - Gilbert Public Schools accepts the following as proof of residency:

  • Current Utility Bill
  • Rental Contract
  • Bill of Sale (within the last 30 days)
  • Notarized Letter of Shared Residency (available in the attendance office)

Current Immunizations - Please contact the school health office with questions regarding immunizations.

Parent/Guardian Driver’s License

Custody Paperwork - Please provide court paperwork and/or notarized proof of guardianship.

 Additional paperwork that would be helpful:

  • Special Education records - Current IEP & most recent psychological evaluation
  • Copy of report card from previous school
  • Withdrawal form and withdrawal grades if starting in the middle of the year
  • Standardized Testing results

 Please don’t forget:

  • Your student will need a physical education uniform and combination lock.  Uniforms and locks are available for sale in our Spirit Store for a total cost of $31.00.  Please send your student with cash or check for these items.
  • School lunch is $2.75 - $3.25 each day.  Please don’t forget to provide your child with lunch money.  For menus and information about school breakfast & lunch programs please refer to “Lunch Menus” on the DRJHS webpage under Parent Zone.
